Karen has known John for a long time now. Even when they were little more than strangers, they communicated well with each other; she still finds it more than a little amusing that they pretended to be married when they first met, long before they started talking about marriage themselves. Since then, she thinks she's become pretty good at reading him. It's been a necessity at times. So when he looks at her like that, she gets what he's trying to tell her, or she believes that she does. As confused as she is and as badly as she'd like to know what's going on, this isn't the time to start blurting things out that only John is aware she knows.
Considering that the woman in their apartment is armed with a shotgun, she probably wouldn't have anyway.
